The Importance of Zinc in PANS and PANDAS: Appetite, Immune Health and HTMA Insights

PANS and PANDAS can change a child’s relationship with food very suddenly. A child who previously ate a wide range of foods may develop intense anxiety around eating, fear of choking, contamination concerns, or a very restricted list of “safe” foods. For parents, this can feel distressing and confusing, especially when nutrition and weight become a concern alongside emotional and behavioural changes.


Nutrition can help support the body during periods of neurological and immune stress. One nutrient that is particularly relevant in this context is zinc. Zinc plays a key role in appetite regulation, taste, smell, immune function, and nervous system stability. When levels are low, eating difficulties can become more pronounced.


How PANS and PANDAS affect eating patterns

Eating changes in PANS and PANDAS are usually driven by anxiety, sensory sensitivity, and neuroimmune disruption rather than typical picky eating.


Children may experience:

• Sudden food refusal or food “fear”

• Restriction to a very small number of safe foods

• Anxiety about choking, contamination, or illness

• Strong reactions to changes in texture, brand, or appearance

• Reduced appetite or weight loss in some cases


Mealtimes can become stressful, with children feeling overwhelmed and parents trying to maintain intake while avoiding added pressure.


Why zinc is important in PANS and PANDAS

Zinc is involved in hundreds of processes in the body, but several are particularly relevant in PANS and PANDAS:

  • It supports immune regulation and helps modulate inflammatory responses.

  • It is essential for taste and smell, both of which can directly affect food acceptance.

  • It plays a role in neurotransmitter balance and nervous system stability.

  • It supports gut integrity and repair, which is closely linked to immune function.


When zinc is low, children may show reduced appetite, heightened sensory sensitivity, restricted eating patterns, and slower recovery from illness or flares. Parents often notice sudden or intense food refusal, especially around taste, texture, or smell changes. Low zinc can also contribute to fatigue, histamine‑type symptoms, anaemia, picky eating, dermatitis, weak or brittle nails and hair, white spots on the nails, poor wound healing, and ongoing gut issues.


Deficiency can develop for several reasons: restricted eating, poor digestion linked to low stomach acid, chronic stress, bacterial imbalance, Pyroluria, or displacement by heavy metals such as arsenic or aluminium.


The role of HTMA in understanding zinc status

Hair Tissue Mineral Analysis (HTMA) offers a clearer view of longer‑term mineral patterns in children with PANS and PANDAS. Blood tests show what is happening right now; HTMA reflects how the body has been storing and using minerals over time. This helps identify whether zinc has been consistently low or whether the zinc–copper balance has been under strain.


HTMA is especially helpful because it shows whether heavy metals may be competing with zinc, which can make the body more sensitive to changes. Since zinc supports detoxification pathways, very low levels paired with higher heavy metals can mean the body responds strongly if zinc is increased too quickly. A low‑and‑slow approach works best here: nutritional support is introduced gradually, at a pace the child can tolerate, guided by the patterns seen on HTMA. This helps support zinc safely without placing extra stress on the body.


By highlighting these longer‑term trends, HTMA helps shape a calmer, more informed approach to supporting mineral needs, rather than relying on guesswork or short‑term symptoms alone.


Getting zinc levels back into balance

When zinc is low, the focus is on steady restoration rather than aggressive correction. In children with restricted diets, this can be challenging, so a gradual and supportive approach is usually best. Food sources of zinc include meat, seafood, eggs, dairy, nuts, seeds, legumes, and whole grains. However, during flares, many children are unable to tolerate or accept these foods consistently, which is where additional support may be needed.


Zinc supplementation

In practice, my preferred forms are zinc picolinate and liposomal zinc. Zinc picolinate is well absorbed and often helpful for children with low zinc or appetite, taste, and sensory challenges. Liposomal zinc is typically gentler on the gut and easier for sensitive children to tolerate, especially during flares. Both can work well depending on the child’s needs and what they can comfortably manage.


A careful, supportive approach

Zinc is not a cure for PANS or PANDAS, but it is an important part of the nutritional picture. Supporting zinc status can help improve appetite regulation, sensory tolerance, and immune resilience over time. Zinc becomes part of a wider strategy to support recovery and stability.
